LGBTQ+ and Addiction: Finding Affirming Treatment
LGBTQ+ and Addiction: Finding Affirming Treatment
Everyone deserves treatment that respects who they are. LGBTQ+ individuals face unique challenges that require culturally competent care.
Why the Disparity Exists
The higher rates of substance use among LGBTQ+ individuals are not caused by sexual orientation or gender identity — they are caused by the stress of living in a society that discriminates. This is known as minority stress:
- Discrimination and stigma: Daily experiences of prejudice increase psychological distress
- Family rejection: LGBTQ+ youth who experience family rejection are 3.4 times more likely to use substances
- Victimization: Higher rates of bullying, assault, and hate crimes
- Internalized shame: Growing up in a society that stigmatizes your identity creates deep psychological wounds
- Bar culture: Historically, bars have been among the few safe spaces for LGBTQ+ socialization, creating proximity to alcohol
What Makes Treatment “Affirming”?
Affirming treatment means providers who:
- Use correct pronouns and chosen names without requiring legal documentation
- Understand the intersection of identity and addiction
- Address minority stress and trauma as root causes
- Include LGBTQ+-specific groups and programming
- Have staff trained in cultural competency
- Create physically and emotionally safe environments
