What Is SBIRT? Screening for Substance Use in Healthcare Settings
SBIRT catches problems early — before they become crises. Early intervention saves lives and reduces healthcare costs.
How SBIRT Works
S — Screening
A quick questionnaire administered in healthcare settings (emergency rooms, primary care, hospitals) to identify patients with risky substance use. Common tools include:
- AUDIT: Alcohol Use Disorders Identification Test
- DAST: Drug Abuse Screening Test
- CAGE: Four yes/no questions about drinking patterns
BI — Brief Intervention
For patients whose screening indicates risky use, a 5-15 minute conversation using motivational interviewing techniques. The goal is to raise awareness and motivation to change — not to diagnose or lecture.
RT — Referral to Treatment
For patients whose screening indicates a substance use disorder, a direct referral to specialized treatment. This is where resources like Red Door Recovery Network become essential — helping connect patients to the right level of care.
Why SBIRT Matters
- Early identification: Catches substance use problems before they become severe
- Cost-effective: SBIRT reduces emergency room visits, hospitalizations, and criminal justice costs
- Evidence-based: SAMHSA and the US Preventive Services Task Force both recommend routine screening
- Reduces stigma: When screening is routine, it normalizes conversations about substance use
