How Contingency Management Works

Contingency management is straightforward: patients receive tangible rewards for meeting treatment goals, such as providing drug-free urine samples or attending appointments. The rewards are usually small — gift cards, vouchers, or prize draws — but the impact is significant.

The science behind CM is simple reinforcement theory: when positive behavior is immediately rewarded, people are more likely to repeat that behavior.

  • CM is the most effective behavioral treatment for stimulant use disorders (methamphetamine and cocaine), according to NIDA
  • The VA has implemented CM nationwide for veterans with stimulant use disorders
  • CM improves treatment attendance and retention across all substance types
  • California’s Medicaid program launched a statewide CM benefit in 2024 — the first in the nation

How CM Is Implemented

  • Voucher-based: Patients earn vouchers redeemable for goods or services for each negative drug test. Voucher values increase with consecutive negative tests
  • Prize-based: Patients earn prize draws for meeting goals. Prizes range from small items to larger rewards
  • Frequency: Testing and rewards typically occur 2-3 times per week during the active phase
  • Duration: Programs typically run 12-24 weeks
